In preparation for your surgical procedure at one of our hospitals, and in compliance with the Joint Commission, you will be required to complete a Preoperative History and Physical examination within 30 days of your scheduled surgical date. This may be completed by either your primary care provider or a provider at the Pre-Admission Testing Center (PATC).
