FY22 Accomplishments
System of Care: Teams from the Hartford HealthCare Cancer Institute and Hartford HealthCare at Home collaborated on a telehealth research project that connects an oncologist to hospice nurses and their patients to improve patient care. The project served six patients and has yielded positive patient and provider experience results.
Innovation Award: Hartford HealthCare at Home received the Health Care at Home Innovation Award at the Connecticut Association for Healthcare at Home annual conference.
