Pursuing gender affirmation can be intimidating and even confusing.
The Gender Health Center offers care managers to help guide patients through the process, providing support and collaborating with the variety of medical specialists involved.
Care management is tailored to the individual needs of each patient, and can include:
- Providing information about our comprehensive Gender Health Center.
- Conducting an initial intake assessment so we understand your personal healthcare needs and goals.
- Connecting you with a primary care provider if you do not already have one.
- Coordinating referrals for gender-affirming surgery and/or hormone therapy.
- Identifying any appropriate support services and coordinating referrals.
- Coordinating insurance coverage.
- Providing connections to community-based resources as needed.
